Order and/or download application packs that contain all the forms and information. Downloaded application forms will need to be printed out onto paper and submitted to the appropriate DVLA address given on the form.

We do not send application packs to addresses outside of Great Britain.

If you are renewing your photocard driving licence with a new photo, and you are unable to use our online services, you can do this quickly and easily at participating Post Office branches. Check if you are eligible to use this service. By using this service, you can expect your driving licence to be issued within 5 days.

Digital Tachograph Card

You can apply online for your first driver digital tachograph card, renew a card (if its expiring within 90 days) or replace a lost or stolen card. Alternatively, you can apply by post using an application for a driver digital tachograph smart card (D777B).

Private (personalised) number plates

You can apply to transfer or retain a private registration number online or you can keep a registration number or put a registration number on another vehicle online. You can also do this by post using form V317 to transfer or retain a vehicle registration number.

Vehicle registration

If you are registering a previously used vehicle, including used imports for the first time in the UK you will need to apply for a first vehicle tax and registration of a used motor vehicle (V55/5). Follow the instructions in form V355/5 on how to fill in form V55/5 to register a used vehicle for the first time.

If you are registering a new vehicle, including new imports for the first time in the UK you will need to apply for first vehicle tax and registration of a new motor vehicle (V55/4). Follow the instructions in form V355/4 on how to fill in form V55/4 for first vehicle tax and registration of a new vehicle.

If you are registering a previously used or new mobility vehicle for the first time in the UK you will need to apply for a first vehicle tax and registration of a mobility vehicle (V55/MV). Follow the instructions in form INF304 on how to register a mobility vehicle.

To get a V5C registration certificate (log book)

You can get a duplicate log book online if you do not need to change anything on the log book. If you need to make changes or you’re not registered as the vehicle’s keeper, you must apply for a vehicle registration certificate by post (form V62).

To register your vehicle off the road – a Statutory Off Road Notification (SORN)

If you’re registered as the vehicle’s keeper you can make a SORN online or phone 0300 123 4321. If you’re not the vehicle keeper you must fill in and post a V890 form to make a Statutory Off Road Notification (SORN). If you have not already applied to be the registered keeper, you must also apply for a vehicle registration certificate (form V62).

The following forms can be requested for postal delivery. We do not send applications to addresses outside of Great Britain.


D2 and D2/D4 pack

You can order the ‘Application for a lorry, minibus or bus driving licence’ – the D2 form, or a Welsh version (D2W). You can order it with or without the D4 medical form.

You can get a D1 from most Post Offices.
